Corporate / Sdn Bhd

Your company name ends with "Sdn Bhd" or "Bhd". Registered with SSM under the Companies Act 2016.

All corporate entities submit the following federal SSM documents:

1. Certificate of Incorporation (Form 9 / Superform / Section 14): your company's incorporation certificate showing the company name, registration number, and date of incorporation.
2. Certificate of Incorporation upon Change of Name (Section 17): required if your company has changed its name since incorporation.
3. Allotment of Shares (Form 24 / Section 78): the share allotment form showing shareholding details.
4. Directors, Managers and Secretaries Particulars (Form 49 / Section 58): the form listing all current directors, managers, and company secretaries.
5. Notice of Situation of Registered Office (Section 46 / Form 44): showing your company's registered office address.
6. Memorandum and Articles of Association (M&A) / Constitution: your company's constitution or M&A document.
7. SSM Company Profile (SSM e-info): the full company profile from SSM, issued within the last 1 to 3 months. Shows corporate information, directors, shareholders, share capital, and financial summary.

West Malaysia

• Authorization letter (Surat Kebenaran Perwakilan Kuasa): a letter on company letterhead signed by the director(s) or key responsible person, or a Director's Resolution authorizing the account setup.

No state-issued trade license is required for West Malaysia.

East Malaysia

Submit your state-issued trade license in addition to the federal SSM documents above:

Sarawak:

• Trade License (Form 109 / Ordinance Chapter 142): issued by your local district or city council (e.g., MBKS, DBKU, MPY).

Sabah:

• Trading License (Form 1 / Form B): issued by your local municipal authority (e.g., DBKK, MPS, MPT).

Sole proprietor / Partnership

Registered as a sole proprietor (one owner) or partnership (two or more owners). Your SSM certificate is Form D (Borang D). In Sabah and Sarawak, some sole proprietors register with the state instead.

West Malaysia

1. Certificate of Registration (Borang D / Perakuan Pendaftaran Perniagaan): issued by SSM for sole proprietors and partnerships. Shows the business name, registration number, and owner details.
2. Business Information (Maklumat Perniagaan): the business information printout from SSM, issued within the last 1 to 3 months. Shows the owner, registered address, branches, and business activities.
3. New Registration Form (Borang A) or Trade Name Approval Form (Form PNA 42): if applicable.
4. Proof of operating address: tenancy or lease agreement, or a utility bill (electricity, water, or internet) issued within the last 3 months.
5. Local Council Business Premise License (Lesen PBT): required if you operate from a physical commercial storefront (e.g., DBKL, MBPJ, MBSA).

East Malaysia

Some East Malaysia sole proprietors register under state ordinances rather than SSM. Submit whichever registration documents you hold. If you have both SSM and state registration, submit both.

All East Malaysia sole proprietors:

• Proof of operating address: tenancy or lease agreement, or a utility bill (electricity, water, or fixed-line telephone) issued within the last 3 months.

Sarawak:

1. Trade License (Form 109 / Form 104): issued under the Trades Licensing Ordinance (Cap. 142) by your local council (e.g., MBKS, DBKU, MPY).
2. Extract of Register of Business Names (Borang 1): extract from your District Office.

Sabah:

1. Trading License (Form B / Form 1): issued under the Trades Licensing Ordinance by your local municipal authority (e.g., DBKK, MPS, MPT).
2. Extract of Business Registration / License Info: if applicable.

Limited Liability Partnership (LLP)

Your business name includes "PLT" (Perkongsian Liabiliti Terhad). Registered with SSM under the LLP Act 2012.

1. Notice of Registration: issued under the Limited Liability Partnerships Act 2012.
2. Certificate of Registration: if applicable.
3. SSM LLP Profile: the LLP corporate information printout from SSM, issued within the last 1 to 3 months. Shows status, registered address, and compliance history.
4. LLP Agreement: the agreement detailing partners' rights, duties, and management structure (if executed).
5. Partnership Account Mandate Form: signed by all partners, specifying authorized signatories.
6. Proof of operating address: tenancy or lease agreement under the LLP or partner's name, or a utility bill (electricity, water, or broadband) for operating premises, issued within the last 3 months.

Association / Society (ROS)

Registered with the Registrar of Societies (ROS). Includes NGOs, clubs, and professional bodies.

Registration documents:

1. ROS Certificate of Registration: the registration certificate issued by the Registrar of Societies (ROS), confirming the society's name and registration number.
2. Constitution / Rules of Society: a certified copy of the society's constitution, detailing its objectives, committee structure, and rules.
3. Certificate of Change of Name: required if your society has changed its name since registration.
4. Return of Office Bearers (Borang 9): the most recent list of current committee members, president, secretary, and treasurer.

Identification and address verification:

1. Office bearers' identification: copy of MyKad (front and back) or valid passport for all principal office bearers (president, secretary, treasurer) and authorized account signatories.
2. Proof of residential address: utility bill or bank statement within the last 3 months for key office bearers, if the address differs from their NRIC.
3. Proof of registered address: tenancy agreement, premise ownership document, or official correspondence showing the society's registered address.

Authorization:

• Meeting minutes / Committee resolution: a certified extract of committee meeting minutes or signed resolution authorizing account setup and designating authorized representatives.
